Only complete this form if you are the parent; please do not complete this form if you are any other relative or friend. ​
You will need to complete this parental consent form if you are the parent of a student who is under18 and you would like your son or daughter to study with us.
You will also need to send us a copy of your passport so we can check that you are the parent.
You can complete the form below or download it and email it to us as an attachment.   
With your child please read the Terms and Conditions for Under 18s and, if your child is under 16 years old also read the information for under 16s. Your child will need to complete the New student Registration form and the level test before we can admit him/her to the school.

    Would you like your child to convert to a programme with more students if this is possible when he/she arrives?
    YOUR SON/ DAUGHTER'S TRAVEL ARRANGEMENTS AND ACCOMMODATION
    Will your son/daughter travel to the UK to study English? Please tell us if he/she will travel to the UK, who he/she will travel with and where he/she will live while they are studying English at ELH?
    MEDICAL AND/OR HEALTH ISSUES
    Are there any medical and/or health issues or concerns we need to be aware of? eg has asthma, has a nut allergy, is allergic to penicillin, must take medication each day and will bring medication with him/her, has braces on his/her teeth etc, etc 
    Please note that if your child has a medical emergency, you need to give your consent for the DOA or the Director ELH to agree to treatment being administered in your absence. Do you agree for us to do this?
    ​If an emergency is life threatening, medical staff can proceed without consent to save your child's life or prevent serious deterioration, but as soon as the emergency has passed, they will require your consent for continued treatment. 
    In an emergency we will always try to contact you and the emergency contact you give us here. This contact must be someone you know and trust who lives in the area and wil be responsible for your son/ daughter while they are in the UK.
    EMERGENCY CONTACT
    ​If there is an emergency we will need to contact the person responsible for your child. Please tell us who this is. 
    Please attach a copy of your passport here. We need to check that you are the student's parent. The copy must show your name clearly. If you prefer you can attach a copy of your passport and send it to us by email.
